TALENT: How-To Complete a Media Request Form

When responding to a Media Request you may come across a request from a Casting Director to complete a form. Forms are used by casting directors to obtain specific information or experience they are seeking for the project. Three important things to keep in mind when completing a media request with a form:

1. In order to access the form you must first confirm the media request. 

2. You can submit your responses to the form without having to upload media to the request! If you upload your media first, you can come back to the request and submit your form responses afterward.

3. Once the form is submitted the responses CANNOT be updated. Please make sure to double-check responses before submitting in order to avoid errors. 

1. When receiving a media request with a form you will notice it will state a form is included in the preview.

2. Upon opening the request you will see the option to confirm or decline the request.

3. Once the request is confirmed you will see the option to complete the form appear. 

4. Click on the title of the form (the blue text under 'FORMS') in order to open the form. Answer the questions and select "Submit Form"

5. Once the form is submitted you may open it to review your answers, but answers cannot be changed.

Your form responses will automatically be sent to the Casting Director!
